Shuttle SN95G5

Those fit inside, but they do take up the one and only PCI slot in the case, so you can't put in a sound card or TV capture card. If you aren't planning on using its PCI slot, they're a nice add-in extra. Hmm... actually, I have a spare one floating around and haven't filled my PCI slot yet...

SFF Recomendations

There are some decent competitors out there now, but Shuttle still sells good cases, and they all come with the 4 items you listed above. There are a few niche markets or special situations, but I don't have a problem recommending shuttle for most SFF uses.
