Just to update everyone, this card was the first recommendation and I through it in today. No issues at all. Happily displaying 2560x1080 on both displays.
Also for future readers this video card is in passthrough mode in esxi 6.0...
Its running one monitor at full resolution now on hd 5450 hdmi (not 2.0)
The dvi port is maxing out on 1920 x 1080 though.
So i would agree its not a problem on a sub hdmi 2.0 connection
I'm going to try the shot solution in the first response with dual hdmi ports first. If that doesn't pan...

edit: I'm thinking hdmi and display port are my best bet with a 4k dp2hdmi converter?
I was worried about DVI specificallly because my current 5450 dvi with hdmi converter is not capable but the hdmi connection is fine. But I suppose thats just the limitation of the cheap hardware?
This isn't going to be for gaming so I don't need high end(read: make it cheap as possible)
I need a single card pci-e x16 that can support two 2560x1080 displays.
For reasons I won't get into, it needs to be a single card and it needs to be AMD.
