9500np Radeon "L shaped" video card for those who missed out

Just a word of caution -- I received mine today from enet, but it didn't have "L" shaped memory. It was the "I" type memory that you see on the 9500 Pro cards.

Pioneer DVR-105 $110 AR @ Fry's

Does anyone happen to have any PDFs or scans of the two rebate forms? I picked up a DVR-105 today at the Manhattan Beach, CA Fry's but they handed me the rebate forms for the DVD+R/+RW drive instead. I didn't notice this until I got home, but I'd hate to drive back to Fry's to get the right...

Does anyone know what hotkey it uses for switching? I wanted to get a Linksys KVM a while back, but I found out that it used CTRL as the hotkey. I use CTRL as my jump key in FPS games, so that kind of ruled it out. :(
