Edit - Sorry I guess I got confused by the 120mm high speed fans I just installed on my radiator being 2200 rpm, but my case fans are 1600 rpm. Also forgot that two of the 140s are not high speed. So it's all solved now :)

Remember the center of the lens is always going to be more sharp than the rest of the image. She looks plenty sharp but maybe a slight overexposure of the skin combined with not being in the center is making you think she's out of focus.
Did you set the date/time in the camera's settings? Does this setting persist through replacing the battery? There is a tiny battery inside the camera usually to keep the time, but this battery could be dead in yours. I think the main battery is supposed to charge that up when it can though.
I'm guessing one reason is that the sensor is so much smaller so any mechanical stabilization system doesn't have to move as much to compensate for hand shake. Phones also have more sensors, like accelerometers, that could help in any stabilization algorithm.
Are you looking at comparable lighting scenarios? Just because they're both night time doesn't mean the ambient light from lamps/other artificial lighting isn't comparable. A lamp pointed in the right direction makes such a huge difference at night.

It's all about light. No webcam is good at night with no lights on. Sit next to a window during the day or point some lamps at yourself and place them close by.
