It depends on whats wrong. If its the Transister it shouldn't be very expensive for the part maybe 50 to 100 bucks and labor should be no more than 30 - 50 bucks an hour depending on your region. I am assuming this is a fairly new 21 inch monitor it would cost you at least 300 bucks to replace...
I used to do monitor repairs and it sounds like it could be a cold solder or a Hsync transister going bad. Since its a 21incher you should take it to a good repair shop not one that will rob you blind. If its a cold solder its a bitch to fix if its an Hsync than its usually easy. Good Luck!

Yeah I forgot you said you had a 5500 the beta drivers will always be better for that. Have you sent the problem to 3dfx tech support. Its probably an issue with the new driver. I'm a little confused by which versions worked for you and which didn't.
Well all I can tell you is to check the version of directX and your video drivers. If you are using the Beta directX 8 drivers you should report the problem directly to microsoft. You should also make sure your not using any Beta drivers for your vid card. Some of 3dfx's beta drivers can be suck.
