Iran has something like 47 million voters. It's not far-fetched that Ahmadinejad won and it's equally not as far-fetched that millions are dissatisfied with the results. Look at how many Republican voters were dissatisfied with the outcome of the 2008 elections, if even a fraction of them were organized and protested, it'd give the impression that the vote was rigged.
