Yeah, you need a permit number to complete the survey. If you don't supply one and you submit, you will be redirected to their home page(like me...doh!) AFter submitting, did you guys get sent to the homepage or did you get a confirmation page saying your shirt will be shipped? If you got sent back to the home page.....it didn't work.

If you didn't put in a permit number....I'm guessing you have to open a claim("2. assign a New Case to obtain your Free Omega Sweatshirt! ) with their firm to get the free shirt. Click on the "bypass survey" and you'll see what I mean. Am I missing something? Thanks,
