I guess Microsoft has had some defective intellimice, and is willing to replace them if you are willing to send proof of purchase and fill out the linked form.

- A copy of your credit card receipt or monthly statement. Please write the Product ID number on the receipt or statement.
- The original label from your box containing the bar code. Cut it from the box and enclose it. Please write the Product ID number on it.
- A photocopy of the bottom of your mouse showing the Product ID number. If the Product ID number is not clearly legible, please write in on the copy.
