Not directly, but you won't be able to kill app if anything happens.
Also some programs lock up doing some infinitive task (for example assign real time to 3D studio max ir run preview, it's looped so system will never finish work and you won't be able to stop it).
High is more than enought for a game (I think you want to use it because of share/ftp access or background apps don't allow to play smooth).
Realtime just means that the game would get all the cpu power it wants, this probably isn't a good idea, as it will take away from some system processes. If you're going to change it, I wouldn't go farther than high.
Choosing real Time will dedicate ALL available time to the activity, do not choose real time unless your using it for a short sequence that you need completed ASAP. You do not choose it for regular programs and games that have no definitive ending point.
It wouldnt cause any harm but your system would pretty much come to a dead halt as all resouces would be dedicated towards the game, leaving little if any resources left to even respond to mouse or keyboard commands.
