Dextromethorphan is an antitussive dissociative. It is the dextro isomer of Racemethorphan, which at doses of 2mg/kg relaxes lung passages much like amphetamines.

relaxing the lung passages is useless if the alveoli are all clogged with water and pus, preventing absorption of oxygen. I don't recall ever prescribing dextromethorphan to treat pneumonia....but then again, maybe i'm just a quack.

That's right, drowing is drowning.

She was so oxygen starved, getting her into a hyperbaric chamber would've been only chance to save her since her lungs were so filled with fluid.

I've never worked with anybody who didn't screw up at some point in time, and given the sheer workload and difficult stituation that health care workers deal with, it's not surprising that they have their share of screw ups. Always have, always will. Anybody who's work in the health industry knows that people make mistakes, cut corners, etc. It doesn't mean they're bad, it means they're human. Some of it is gross negligence, for sure, but there is not a doctor working now who's not made a mistake (unless it's his first day).

It could be just a mistake, but it's also my opinion that increased involvement of insurance companies and Medicare in limiting Doctor's and hospital's ability to operate whatever way they think is best for healthcare, in favor of controlling costs..

has created a culture where treating the patient is almost secondary to making sure that all the paperwork is right so the Doctor/hospital don't get shafted by Medicare/Insuance company.

I am a programmer, and I do occasionally run into a client who aggrandizes his intelligence and expertise into things they have little clue about while railing away about some problem. I correct the error if I can, free of charge if the situation warrants. No it is not the doctor's job to play god with everyone they see. It is their job to play scientist. They look at evidence thoroughly and come to a conclusion. This is different than making random guesses or arbitrary decisions.

Like a programmer, a doctor should have certain methods in place to prevent such outcomes. I don't know about you, but I've had bronchitis AND pneumonia several times in my life. It is not difficult to tell the difference even as the kid who HAS the symptoms. In every case, I was hospitalized at least for several hours and monitored. I don't recall ever being told to take tylenol for a lung disorder.
We'd love to think it's as simple as that, don't we? Of course, far too often doctors are working off of mere observations and inference instead of direct scientific evidence regarding what ails a patient. Sooner or later there will be a misdiagnosis, and I for one will not scream, "Lawsuit!" and let that become my natural reaction. Frankly I find that more than a little pathetic.
