Please help with warranty.

I have an ATI 9700 Pro that needs an RMA. I am trying to fill out the online "Built by ATI" RMA form, but have trouble with some numbers.
The adress where the form can be found is https://apps.ati.com/rma/rma.html

The only two labels I see on the card are as follows:

1) Sticker with bar code that says "S/N:HXXXXXXXXXX" (Xs represent numbers)

2) Sticker with bar code that says "R97-C3G" above the bar code and the number "47108XXXXXXXX" (Xs represent numbers) below it

Obviously number 1 is the serial number, but I am having trouble with the "102 Product Part #". I looked at the help ATI provides on their site on finding these numbers. But that didn't help as I have no number that starts with 102.

There is another number on the card itself (not an sticker) that says "PN 109-AXXXXX-XX" where the Xs represent numbers. But then again the site says that these numbers are found on stickers....

Any help regarding the Product Part Number will be greatly appreciated.

call them and find out what numbers you need. its best to be sure with an RMA

# Select Start > Programs > Accessories > System Tools > System Information.
# Click the [+] sign next to 'Components'
# Highlight 'Display'
# For Windows 98/98SE/ME/XP: Click "File", then select "Export".
For Windows 2000: Click "Action", then select "Save as Text File...".
# Type a name to save the file.

Look at the saved file and use the PNP Device ID. ATI can tell what you have by that number string

Google the "R97-C3G" and it appears to be a Powercolor card not a built by ATi card. To further confirm it google the 109-AXXXXX-XX part number and see what you get.
