It depends on the mounting, I guess. If it's an addon bracket that came with the case, I'd say it would depend on the length and weight of the card, and how much length and support the mounting bracket offers. An additional bracket would be a good idea if the vertical mounting bracket is short, thin, etc.
I assume you're referring to a standard tower with a vertical GPU mount option, and not a case where the mobo lays flat (like a cube case).
I suppose I should have asked what kind of anti-sag bracket you had in mind. I forgot that there's a few different types. I had used a simple adjustable bolt type that the end of the card rested on. There are also extended and reinforced brackets too that mount where the GPU screws down in the case slots. Never used one of those myself.

^ this, unless OP has some very unusual setup.Does your GPU look like it's sagging? I'd be surprised if you need one when vertically mounting a GPU. The same forces do not apply when mounted in this way. The GPU can not pivot around the PCIe slot like is does when mounted horizontally.
